29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    (a)  Employees were using pneumatic air guns and exposed to serious eye hazards without wearing safety eyewear.      (b)  Employees were working and traveling below the work levels of the roof with out donned head protection.    (c)  Employees were found to have access to defective fall protection devices, such as an activated shock absorber, which would inhibit adequate protection from falling.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Employees conducting residential roofing activities on a 12:12 pitched residential roof with a 12:12 pitch were not protected from falling to the lower level.  This exposed employees to a fall hazard of approximately 21 feet.
