In the Southeast corner of the Union Station construction site in Springfield, MA: On or about July 8, 2015, the employer did not ensure the acetylene gas was released from the regulator before an employee started to remove the regulator from an acetylene cylinder.

In the Southeast corner of the Union Station construction site in Springfield, MA: On or about July 8, 2015, the employer did not ensure that a torch, shutoff valves and hose couplings were inspected at the beginning of the shift during which an acetylene tank fire resulted exposing employees to a fire hazard.

29 CFR 1926.352(e): Additional fire prevention precautions were not provided when welding, cutting, or heating operations were such that normal fire prevention precautions were not sufficient: In the Southeast corner of the Union Station construction site in Springfield, MA: On or about July 8, 2015, one employee performing fire watch duties did not guard against fire during, or for a sufficient time after the completion of work, which resulted in an acetylene tank fire which resulted in the torch cutting employee receiving burns to the face and melting of the tank's fusible plugs and valve hand wheel.
