29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  Construction site - On or about July 29, 2015 the employees installing roof decking on two story custom home did not have fall protection.  The employer failed to prove infeasibility/greater hazard.   (b)  Construction site - On or about July 29, 2015 a 56" wide window opening on 2nd floor 11 ' above ground did not have guardrails in place.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards, which would enable each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards and the requirements contained in this subpart:  a)  Construction site - On or about July 29, 2015 employees were exposed to fall hazards and the employer had not provided a fall protection training program for residential construction.
