This employee was approximately 3 feet from this edge when he climbed onto the hopper.      Violation occurred 7-23-15

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    Employees were performing roofing operations on a flat roof of the 4th story roof surface  with a fall of 15 feet to the lower level.  The employer stated that the employees were protected by a warning line system.  However, on the East side near where the gravel hopper was being used, the warning line was approximately 4 feet from the edge.  Also, on the South side of the roof edge, the warning line was also 4 feet from the edge.      Violation occurred 7-23-15

29 CFR 1926.1428(a)(3): 29 CFR 1926.1428(a)(3): The employer did not make the documentation for whichever option is used available at the site while the signal person is employed by the employer.  The documentation did not specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section.    a)   Worksite, 305 Madison Ave., Morristown, NJ     Employees were distributing gravel hoisted to the 4th story roof surface with a Liebherr LTM-1090 110 Ton crane.  The operator estimated the heaviest load was approximately 3000 to 4000 lbs.  The foreman acting as signalman using hand and radio signals said that he was aware of crane signals, however did not have formal training in crane signals and there was no documentation of his  hand or radio signaling training on the site.
