29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a. Site- Employees were not protected from crushing and struck by hazards where the employer failed to obtain JLG Industries Inc. permission to use homemade 12 foot fork extensions with their Lull 944E-42 rough terrain forklift. The fork extensions were found in operation with a bow of 1 1/4 inches near the back third of the fork extensions due to overloading and or tip loading.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a. Site- The employer failed to provide powered industrial truck operator training for operators who showed evidence of training from a past employer. The employer failed to ensure that the previous training was complete including a performance evaluation and that the employees were trained to operate same JLG all terrain forklifts and Nissan forklift which are operated at the site.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Site- Employees operating the extruder are not protected from laceration, crushing, and amputation hazards where the hold downs and dogs cycle into place after being tripped and are not guarded from contact. On or about 5/19/2015 an employee operating the extruder had the tip of their left ring finger caught between a hold down on the extruder and the wood framing being assembled by the extruder, removing a part of the finger tip.
