29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration:    (a)  Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022:  On and before July 1, 2015 the employer did not ensure a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration.  Employees erected a scaffold section that was two-frames wide, two-levels high in front of a Starbucks store without the supervision of a qualified competent person.  This condition exposed the employees to a fall hazard of approximately 10 feet, 9 inches.

29 CFR 1926.451(g)(1):  Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level:    (a)  Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022:  On and before July 1, 2015 the employer did not ensure that employees were protected from a fall from elevation in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffold where two employees were applying siding and signage to a Starbucks store front. This condition exposed the employees to a fall hazard of approximately 10 feet, 9 inches.

29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    (a)  Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022:  On and before July 1, 2015, the employer did not ensure the working platforms on the fabricated frame scaffold were fully planked, in that two employees were applying siding and signage to a Starbucks store front while standing on only one of three planks. This condition exposed the employees to a fall of approximately 10 feet, 9 inches.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.    (a)  Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022:  On and before July 1, 2015, the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system, in that two employees were engaged in siding removal and installation that involved accessing the flat roof of a Starbucks coffee building without fall protection means. This condition exposed the employees to a fall hazard of approximately 18 feet.
