29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: (a) Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022: On and before July 1, 2015 the employer did not ensure a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration. Employees erected a scaffold section that was two-frames wide, two-levels high in front of a Starbucks store without the supervision of a qualified competent person. This condition exposed the employees to a fall hazard of approximately 10 feet, 9 inches.

29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) Indeco Iberia at 10339 Tower Road, Commerce City, CO 80022: On and before July 1, 2015 the employer did not ensure that employees were protected from a fall from elevation in that guardrails or other suitable fall protection were not properly installed on the working platform of the scaffold where two employees were applying siding and signage to a Starbucks store front. This condition exposed the employees to a fall hazard of approximately 10 feet, 9 inches.

This condition exposed the employees to a fall hazard of approximately 18 feet.
