29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in equipment or processes that presented a new hazard:  (a) On or about June 23, 2015 for an employee exposed to a flash fire due to a natural gas vent valve that was left open after releasing a compressor from lockout where the energy control procedure had been changed, but the authorized employee was unaware of the recently added step at or near 14402 36th Street North in Alexander, North Dakota.

29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy:     (a) On or about June 23, 2015 for an employee exposed to a flash fire where the employee who locked out a natural gas compressor days before the maintenance was performed did not transfer the keys to the authorized employee at or near 14402 36th Street North in Alexander, North Dakota.
