29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Drainage On North Atherton Street, Between Martin Street And West Aaron Drive - Employees working in an excavation were exposed to a caught-in hazard from not having an appropriate egress in an excavation that was at least four (4) feet in depth, on or about July 28th, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: a) Drainage On North Atherton Street, Between Martin Street And West Aaron Drive - Employees working in an excavation were exposed to a struck-by hazard from undermining the sidewalk by approximately fourteen (14) inches, on or about July 28th, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
