29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors: (a) Room 395, Reception Area: On or about July 30, 2015 and at times prior, employees were exposed to electric shock/fire hazards by using a 120 volt Relocatable Power Tap that was run through the lower part of a cubicle partition. THE U. S. DEPARTMENT OF VETERANS AFFAIRS, - VA PALO ALTO HEALTH CARE SYSTEM WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 744302, CITATION 01, ITEM NUMBER 004 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 31, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT LIVERMORE CAMPUS 4951 ARROYO ROAD, LIVERMORE, CALIFORNIA 94550
