29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b): a) At 21 Vann Court Millbrook, Alabama: On or about July 29, 2015 and at times prior; the employer failed to ensure that employees installing shingles on the 8:12 pitch roof were provided and were using fall protection exposing them to falls of 8.2 feet to the ground below. THE LAWRENCE LEE DBA LEE CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 432452, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 26, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 141 TURTLE DRIVE MILLBROOK, ALABAMA.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 21 Vann Court Millbrook, Alabama: On or about July 29, 2015 and at times prior; the employer failed to ensure a ladder used by employees to gain access to the roof of a new residential home extended at least three feet above the landing area exposing employees to falls of 8.2 feet. THE LAWRENCE LEE DBA LEE CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(B)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 432452, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 26, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 141 TURTLE DRIVE MILLBROOK, ALABAMA.
