29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Ben Blackwell dba Reliable Roofing and Siding Systems, Inc. had employees exposed to fall hazards without fall protection, on a residential construction site in Mahomet, Illinois.  The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards of falling and the safety requirements associated with fall protection, as well as the proper work procedures to follow, in order to minimize and eliminate the fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       Ben Blackwell dba Reliable Roofing and Siding Systems, Inc. had employees exposed to 9.5 ft. and 12.5 ft. fall hazards while exiting and accessing the roof via extension ladders that did not extend at least 3 ft. above the roof surface, on a residential construction site in Mahomet, Illinois.  These extension ladders included the following:      a. 2.5 ft.  extension  west side of roof on front side and   b. 2 ft. extension  east side of roof on front side.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:     Ben Blackwell dba Reliable Roofing and Siding Systems, Inc. had employees exposed to 9.5 ft. and 12.5 ft. fall hazards while exiting and accessing the roof via extension ladders that did not extend at least 3 ft. above the roof surface, on a residential construction site in Mahomet, Illinois.  The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards related to ladders, as well as the proper work procedures to follow, in order to minimize and eliminate theses hazards.  This training would include:    a.         The nature of fall hazards in the work area,  b.         The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used, c.         The proper construction use, placement, and care in handling of all ladders, d.         The maximum intended load carrying capacities of ladders, and  e.         The standards contained in this subpart, Subpart X  Stairways and Ladders.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      Ben Blackwell dba Reliable Roofing and Siding Systems, Inc. had employees exposed to fall hazards without fall protection, on a residential construction site in Mahomet, Illinois.  These fall hazards included:      a. 9.5 ft.  lower roof section and   b. 12.5 ft.  higher roof section.    Reliable Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 1060728, citation number 1, item number 1b and was affirmed as a final order on May 20, 2015, with respect to a workplace located at 2852 S. 11th Street, Springfield, IL.
