Royalton Ohio 44133, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to cave in hazards where employees were working in a trench with varying depths of 6.7 to 8.3 feet.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about July 31, 2015, at a job-site located at 8365 Royalton Rd, N. Royalton Ohio 44133; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth varying from approximately 6.7 to 8.3 feet. The trench was determined to have type "C" soil.
