29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    Hearthside Food Solutions, LLC - McComb, Ohio: On or about July 24, 2015, lockout devices were not affixed to the energy isolating device for the Line 8 cartoner machine (s/n 200 162) before employees serviced the machine's chain which had become displaced.  A potential for unexpected startup of the machine existed while an authorized employee serviced the chain drive.

29 CFR 1910.147(e)(2)(ii): After lockout or tagout devices were removed and before machines or equipment is started, affected employees were not notified that the lockout or tagout device(s) have been removed:    Hearthside Food Solutions, LLC - McComb, Ohio: On or about July 24, 2015, employees in the work area of the Line 8 cartoner machine (s/n 200 162) were not notified that lockout devices were removed from the machine prior to the machine being restarted after maintenance/servicing activities.  An employee working to clean waste from the floor of the area was exposed to moving parts following the machine's start-up.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:    Hearthside Food Solutions, LLC - McComb, Ohio: On or about July 24, 2015, the employer did not ensure that the sprocket used to power the compression belt and associated chain and sprocket driven by the motor for the flites on the line 8 cartoner machine (s/n 200 162) were guarded to protect employees from being caught in the rotating parts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Hearthside Food Solutions, LLC - McComb, Ohio: On or about July 24, 2015, the employer did not ensure that employees were protected from the in-going nip point located at the discharge end of the Line 8 cartoner machine (s/n 200 162).  Employees were exposed to rotating parts and in-going nip points created by the conveyor belt and pulley.      Hearthside Food Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 313786212, citation number 1, item number 1 and was affirmed as a final order on January 21, 2011, with respect to a workplace located at 312 Rader Rd. in McComb, Ohio 45858.

Hearthside Food Solutions, LLC - McComb, Ohio: On or about July 24, 2015, the employer did not ensure that lockout devices were marked to identify the employee applying the lock.  One lock without identifying information was used during a time two maintenance workers serviced the Line 8 cartoner machine (s/n 200 162).
