29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable.   Autotech Shops - The employer did not establish an energy control program that consists of energy control procedures, employee training and periodic inspections for student apprentices performing routine maintenance and service on various types of motor vehicles.  Types of stored energy include, but are not limited to, mechanical, gravitational, hydraulic, and electrical.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.   Autotech Shop(s) - Student apprentices were not trained by the employer to be authorized in LO/TO procedures.  Student apprentices perform maintenance and repairs on motor vehicles placed on auto lifts and are potentially exposed to the unexpected release of energy types such as, but not limited to hydraulic, mechanical,  gravitational, and electrical of various magnitudes.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:   Autotech Shop - A Dodge Ram 1500, 4x4 pickup placed on an AMMCO 4-post lift for maintenance work, was not restrained or otherwise rendered safe by using methods such as, but not limited to, placing chocks under the tires, or by engaging the emergency brake, or removing and controlling the ignition key,  to prevent the unexpected release of stored mechanical energy.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck by hazards from falling loads, vehicles and/or lift parts:   a.    Autotech Shop:  On or about June 23, 2015, employees were exposed to crushing and struck-by hazards in that the employer failed to perform and document daily and periodic inspections of the AMMCO 4-post surface mounted lift as required by the AMMCO Installation and Operation Manual and the ANSI standard (ANSI/ALI ALOIM:  2008).     b.    Autotech Shop:  On or about June 23, 2015, employees were exposed to crushing and struck by hazards in that the employer failed to ensure and document that employees were trained in the safe use and operation of the AMMCO 4-post surface mounted lift as specifically required by the AMMCO Installation and Operation Manual and the ANSI Standard (ANSI/ALI ALOIM: 2008).  c.    Autotech Shop:  On or about June 23, 2015, employees were exposed to crushing and struck by hazards in that the employer failed to display, for employee use and information, the AMMCO Installation and Operation Manual; the Automotive Lift Institute's safety manual,  "Lifting it Right"; the Automotive Lift Institute's "Safety Tips Guide"; and the ANSI Standard (ANSI/ALI ALOIM:2008)- "Standard for Automotive Lifts - Safety Requirements for Operation, Inspection, and Maintenance", and in the case of frame engaging lifts,  "Automotive Lift Institute's Lifting Points Guide", in a conspicuous location in the lift area convenient to the operator(s).     Among other means, some feasible and acceptable methods to correct these hazards would be for the employer to:    1.    Establish an automotive lift inspection and maintenance program as required by ANSI Standard ANSI/ALI ALOIM:2008 Section 5.1 which states: "The owner or employer shall establish a periodic inspection procedure in accordance with the recommendations of the lift manufacturer in order to ensure reliability and allow the continued safe operation of the lift".    2.    Conduct daily inspections as required by the AMMCO Lifts Installation and Operation Manual, Section 5.1 which requires Inspect the lift daily.  Do not operate if malfunctions occur or damaged parts have been found.    3.    Document the periodic inspections as required by ANSI Standard ANSI/ALI ALOIM:2008, Section 5.4 which states A record of each periodic inspection shall be prepared and maintained noting the observations and findings of all points of inspection recommended by the manufacturer as well as subsequent repairs or replacements accomplished.  4.     Provide training to employees on the proper use and care of the lifts in accordance with the lift manufacturer's Installation and Operation Manual(s) which requires that only trained and authorized personnel should operate the lift.    5.      Provide training on the proper use and care of the lifts as required by ANSI/ALI ALOIM:2008, Section 4.2.1, which states: The owner or employer shall ensure that operators of automotive lifts are instructed in the safe use and operation of the lift using the manufacturer-provided instructions and warning labels and the Automotive Lift Institute publications, "Quick Reference Guide", "Vehicle Lifting Points for Frame Engaging Lifts", "Lifting it Right",  "Safety Tips" and this ALOIM Standard.    6.     Document the training as required by ANSI/ALI ALOIM:2008, Section 4.3, which requires that the owner or employer shall appropriately document operator training by completing the operator training log found in the appendix to the standards.  7.     Maintain and display the AMMCO Installation and Operation Manual; the Automotive Lift Institute's safety manual, "Lifting it Right"; the Automotive Lift Institute's "Safety Tips Guide"; and the ANSI Standard (ANSI/ALI ALOIM:2008) - "Standard for Automotive Lifts - Safety Requirements for Operation, Inspection, and Maintenance"; and the Automotive Lift Institute's "Lifting Points Guide", in a conspicuous location in the lift area convenient to the operator(s) as required by ANSI/ALI ALOIM:2008 Section 4.2.2 which states:  ANSI/ALI ALCTV requires the lift manufacturer to supply operating instructions, general safety information, safety tips and warning labels with each lift manufactured.  The owner or employer shall display these materials in a conspicuous location in the lift area.
