29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch: a) In the Back Shop area, east side, a direct drive 40 inch by 40 inch box fan, located on the floor was guarded with a wire mesh that had openings that were 1 inch by 2 inches. Additionally, there was a front side 2 inch by 5 inch opening and a back side 2 inch by 6 inch opening in the mesh. b) In the Back Shop area, west side, a direct drive 40 inch by 40 inch box fan that had rollers, located on the floor, was guarded with a wire mesh that had openings that were 1 inch by 2 inches. There was an additional 1 inch high, 23 inch wide slot between the guards. c) In the Back Shop area, mid-section, a direct drive fan 48 inch diameter was guarded with chicken mesh wire that had openings of 1 inch.
