29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) Worksite: Employees are required to provide repair and maintenance on equipment such as, but not limited to a   Holzher Uno 1303 Edge Bander with numerous sources of energy. The employer did not provide the employee with a Lockout/Tagout program; on or about 7/31/15.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.213(b)(5): On each machine operated by electric motors, positive means was not provided for rendering such controls or devices inoperative while repairs or adjustments are being made to the machines they control.   a) Workplace, 55 Edison Ave. West Babylon NY - Employees operating a Holtzer Uno 1303 Edge Bander, were adjusting the timing of the cuts while the machine was running.  The positive means for rendering the controls or devices inoperable was missing; on or about 7/31/15.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
