29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  On October 8, 2015, , Joe Sieg dba Extreme Home Improvement, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:      a.    Citation 1 Item 1a & b and, b.    Citation 2 Item 1.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard of approximately 14 ft 9 in. on a construction site in Peoria, Illinois and adequate fall protection was not being used.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:     Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard of approximately 14 ft 9 in. on a construction site in Peoria, Illinois and the warning line systems were not erected at least 6 feet from the roof edge.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:     The employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.     Joe Sieg dba Extreme Exteriors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 108657, citation number 2, item number 1 and was affirmed as a final order on January 24, 2012, with respect to a workplace located at 300 West Detwiler Drive, Peoria, IL 61614.
