29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: A.) ADESA, Inc. dba ADESA Pittsburgh/ 785 Franklin Road Mercer, PA: On or about July 27, 2015 and at times prior there to, an employee hospitalization was not reported to OSHA within 24 hours. The accident and resulting hospitalization took place on or about July 10, 2014. The incident was not reported to OSHA until July, 24, 2015.
