29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  Employees were exposed to electrical shock while working around exposed electrical parts with a voltage of 120/240 volts. 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    a)  Employees performing roofing work were exposed to fall hazards of greater than 25 feet.  A Good Reliable Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 63041, citation 1, item 1, and was affirmed final order on or about November 11, 2011, with respect to a workplace located at 11125 Wisconsin Ct., Orland Park, IL 60467.    A Good Reliable Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11) which was contained in OSHA inspection number 730601, citation 1, item 2a, and was affirmed final order on or about January 10, 2013, with respect to a workplace located at 15100-15126 Woodfarm Rd., Plainfield, IL 60544.    A Good Reliable Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 804201, citation 1, item 1, and was affirmed final order on or about March 11, 2013, with respect to a workplace located at 2328 S. Kedzie Ave., Chicago, IL 60623.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
