29 CFR 1926.452(n)(1): Scaffold platforms were placed higher than the second highest rung or step of the ladder supporting the platform: (a) Left-front side of the building - On or about August 4, 2105, the platform of the step ladder scaffold system was placed on the top step of the left side ladder. An employee was installing Hardie Board siding materials from this system. Elevation of the work plank was approximately 7 feet from the ground.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) Left side of the building - On or about August 4, 2105, the employee that was installing fascia materials to the gable end of the building was not wearing a body belt or a harness while performing this work from an articulating boom man lift. Elevation was approximately 18 feet from the ground.
