29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    Employees dumping roofing debris were exposed to a fall hazard of approximately 15ft 9in in Springfield, Illinois, because a guardrail system or other fall protection system was not being used to protect the employee from falling off the roof.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:    The employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards, when working on a low sloped roof.  There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.     Blackhawk Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1049637, citation number1, item number 2B and was affirmed as a final order on May 28, 2015, with respect to a workplace located at 1101 E. Monroe St. Springfield, IL 62703.
