29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: A.) Great Lakes Insurance Services Group, LLC, 3445 West 12th Street, Erie, PA 16505, north side of the structure, on or about August 4, 2015 and time prior thereto: An employee was performing work tasks including, but not limited to, cutting and shaping wood shingles around the window casings, from the basket of a JLG 340AJ aerial lift at a height of 12.2-feet. The employee was not utilizing a personal fall arrest harness and lanyard system.
