29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    a) Uticom, Inc. 109 Independence Way, Coatesville, PA (Screen Printing, Laminating and Assembling areas) - The employer did not have a written certification of the hazard assessment. The employer did not have each affected employee wear appropriate personal protective equipment such as gloves, face shield, apron, footwear etc. to protect their eyes and skin from chemicals used for screen printing process, such as, but not limited to, sodium hydroxide found in Liquid Haze Remover, which is corrosive (pH is greater than 13). Observed on or about August 4, 2015.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a)  Uticom, Inc. 109 Independence Way, Coatsville, PA:  A written hazard communication program was not developed, implemented, and maintained at the workplace where employees were exposed to chemicals such as, but not limited to, cyclohexanone contained in Techmark TM series solvent based inks, 2-Butoxyethanol contained in Nazdar 9700 series All Purpose Screen Ink, Toluene contained in Quaker City Chemicals Lacquer Thinner, DL-10, sodium hydroxide contained in Liquid Haze remover, etc.  Observed on or about July 30, 2015.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) Uticom, Inc. 109 Independence Way, Coatesville, PA - The employer did not ensure that secondary containers used for storage and transportation of chemicals including, but not limited to, Sodium Periodate contained in ICC 833 Stencil Remover and Sodium Hydroxide contained in ICC 924 Liquid Haze Remover, were labeled, tagged or marked with the product identifier and words, pictures, symbols or combination thereof.  Observed on our about August 4, 2015.
