29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to the hazards of burns, smoke inhalation and death from fire related causes: Frog Fire, Modoc National Forest, CA: On July 30, 2015, the U.S. Forest Service failed to implement procedures to ensure Fire Fighters can safely evacuate the area. While fighting a fire, the winds shifted, pushing the fire toward a Fire Fighter resulting in a burn over. Implement procedures to ensure Fire Fighters know the escape routes and to maintain communications with all firefighters when evacuating a fire. When communications is lost, implement appropriate search and rescue procedure.
