29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a) The machine specific procedures did not clearly and specifically outline the techniques to be utilized for the control of hazardous energy for equipment in the facility, including but not limited to the SMB Flexographic Printing Press  number 6 machine and the Shirring Machine. These techniques include but are not limited to the type of isolation device for the specific energy source and how to properly verify deenergization of the energy source(s). Hazardous energy sources include but are not limited to electrical, pneumatic, and gravity.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operation cycle(s):  (a) SMB Flexographic Printing Press #6: The printing press did not have the point of operation guarded during operation and cleaning cycles. Employees were able to access the point of operation created by the ingoing printing cylinders, exposing employees to amputation, crushing, and caught-in hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Employees have not received hazard communication training, to include training on the new global harmonization system requirements and information on the chemicals they handle, such as but not limited to PM-Acetate and ethyl acetate.
