29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Job Site - On or about August 5, 2015, employees were exposed to fall hazards of approximately 9 feet while working on the 2nd floor of the residential home installing plastic piping without fall protection such as, but not limited to, a guardrail system in place.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: a) Job Site - On or about August 5, 2015, employees were exposed to fall hazards up to 9 feet in height due to accessing the stairway with material and equipment to work on the 2nd floor of the residential home without a stairrail system in place.
