29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: a) Route 46 @ Mile Marker 2, Knowlton, NJ: Employees were allowed to perform retaining wall installation operations approximately 15' from the ground below on a 14' 2" foot long scaffold platform resting on the adjacent rock and a pipe spanning the steel beams, which was not secured to prevent cantilevering. Condition occurred on August 4, 2015.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Route 46 @ Mile Marker 2, Knowlton, NJ: Employees were allowed to use a ladder that was not secured and did not extend 3 feet above the working surface to allow for safe access on and off the scaffold platform. Condition occurred on or about August 4, 2015.
