29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from safety cans, by gravity through an approved self-closing valve: a) On or about 08/05/15 in the Mask Room; employer failed to protect employees from a flash fire when isopropyl alcohol ( flammable ) was dispensed from the 5 gallon safety container by the use of a gate valve. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.334(d): Where flammable materials were present only occasionally, such as but not limited to flammable gases, vapors, or liquids; combustible dust; and ignitable fibers or flyings, electrical equipment capable of igniting them shall not be used. a) On or about 08/05/15 in the Model shop; employer failed to protect employees from fire and explosion hazards when they allowed an ordinary shop vacuum with an arcing electric motor ( Dayton brand - model 4YE60 and 4YE65) to pick up particles of aluminum ( combustible material ) from the milling and sawing equipment as well as floor areas. ABATEMENT CERTIFICATION REQURIED
