29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 5, 2015, at a worksite located at 6973 Pearl Road, Middleburg Heights, OH during commercial roofing activities, the employer did not ensure subcontractor employees were protected from fall hazards approximately 8 feet, by ensuring a guardrail system, safety net system, or personal fall arrest system was being used.
