29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    (a) Pooler residential construction site, on or about and at times preceding to March 31, 2015, for employees who were performing masonry work, base plates were not utilized to secure polls on tubular frame scaffold and the frames were set up on wooden block, exposing employees to a 13-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  (a) Pooler construction site, on or about and at times preceding to August 4, 2015, tubular welded frame scaffold was not equipped with an access ladder for employees laying brick at elevations of 13 feet.
