29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: Gannett Publishing Services, Sheboygan Press facility: On August 4, 2015 and prior to this date, employees were exposed to falls of 10 feet to concrete below located in the pressroom. The employer had installed caution tape around the perimeter of the floor hole but did provide conventional or personal fall protection to prevent the fall exposure.
