OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards due to not providing rope grabs with an anti panic feature:    a. On or about August 05, 2015, at the above located workplace, employee(s) performing window washing operations were exposed to the hazards associated with falls. The employer's fall protection program and training failed to ensure that employees were following the employer's policies and procedures with regard to fall protection. An employee was injured after falling approximately six (6) stories while descending on a rope descent system.
