OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    On or about August 3, 2015, the safety latch of Power Vane Chain Hoist, Model number: CP 1200, TANHAB; Serial number: 94322B was inoperative and employees were exposed to crushing and struck by hazards while loading/unloading bags of chemicals to be used in the mixer #22.
