29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Roof, Right Side of Garage - On or about August 6, 2015, an employee used an aluminum extension ladder to gain access to the roof of the house without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support, exposing the employee to fall hazards of up to approximately 17 feet, 5 inches onto concrete and earthen ground.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: a) Roof, Right Side of Garage - On or about August 6, 2015, the right side rail on the unsecured aluminum extension ladder used to gain access to the roof was not positioned against the structure, thus exposing the employee to a fall hazard of up to approximately 12 feet inches onto concrete and earthen ground.
