29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Tuscaloosa Apartment 10th Ave. and 15th Street: On or about 08/03/2015, a 100 ft. extension cord being used by an employee to operate a sewer camera in order to clear a new drain had a missing ground pin.
