29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit."   a)      This violation was observed on or about August 4, 2015, the exit door on the West wall next to electrical did not have a functioning  Exit sign exposing employees to the hazards of delay in exiting the building in case of an emergency.    b)      This violation was observed on or about August 4, 2015, the exit door on the east side of the warehouse did not have an Exit sign exposing employees to the hazards of delay in exiting the building in case of an emergency.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  This violation was observed on or about August 4, 2015, in the warehouse, employees who had not been trained in the safe operation of powered industrial trucks, were operating pallet jacks in the forward mode with loads higher than the employee could see over exposing employees to a struck-by hazard.

This violation was observed on or about August 4, 2015, the SDSs were not available for hazardous chemicals such as, but not limited to, Nason synthetic enamel reducer, Nason black paint, Bird Dog leak detector exposing employees to the hazards of chemical exposure.
