29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: a) 3448 Veterans Blvd., Metairie, LA; On or about August 4, 2015, the employer allowed employees to work from a scaffold that did not have an adequate firm foundation. Some of the scaffold legs were on scrap wood pieces.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) 3448 Veterans Blvd. Metairie, LA; On or about August 4, 2015, the employer failed to ensure a scaffold used to apply stucco to the exterior of a building was equipped with a ladder for access and egress. The scaffold was 3 levels approximately 21 feet high. Dixie Wall Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 966317, citation number 01, item number 002 and was affirmed as a final order on September 17, 2014, with respect to a workplace located at 4613 Marseilles PL., Metairie, LA.
