(b) The Mitsubishi forklift was missing the propane tank indexing pin. This pin is to assure proper positioning of the rotation of the cylindrical propane tank valve to prevent fuel release if the safety valve were caused to releave internal pressures.

An employees breaking and building pallets was exposed an 8-hour time weighted average sound level of approximately 86.8 dBA or equivalently a dose of 64.6 percent during a 354 minute sampling period.
