Roof- Employees working from the roof were not protected from falls of approximately 15 and 19 feet where fall protection equipment was not properly installed or utilized. One employee was working from the roof with personal fall arrest equipment that was not adequately installed; a second employee was working from the roof and was not protected by any type of fall protection.
