29 CFR 1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support:   (a) River Park Apartments - On or about August 4, 2015 employees gained access to the upper roof by using an aluminum extension ladder that was not properly secured at the top to keep it from being displaced during use.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:   (a) River Park Apartments - On or about August 4, 2015 employees were using the aluminum ladder to gain access to roof that was placed on a 4/12 pitch slopped roof. The ladder was not secured to prevent displacement exposing employees to a fall hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   (a) River Park Apartments - On or about August 4, 2015 employees gained access to the roof using an aluminum extension ladder with a damaged step exposing employees to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) River Park Apartments, Building No. 8 - On or about August 4, 2015 employees were exposed to a fall hazard when applying roofing material on a three story building.  Employees were wearing safety harnesses. The safety harnesses were not attached to lanyards secured to an anchor point.  One employee was working on roof without wearing a safety harness and lanyard attached to an anchor point.
