29 CFR 1910.178(g)(2): Facilities shall be provided for flushing and neutralizing spilled electrolyte, for fire protection, for protecting charging apparatus from damage by trucks, and for adequate ventilation for dispersal of fumes from gassing batteries. The eyewash required by this part was not adequate, in that it did not meet the flow requirements for eyewash units to properly was electrolyte from the eye in the event of an incident in the forklift battery charging area.

29 CFR 1910.178(l): Operator training. 29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. The employer failed to document and certify forklift operator evaluation under 29 CFR 1910.178(l)(4)(iii) for the required evaluation of operator performance for employees trained in forklift operation every 3 years.
