Garden City NY - Employees were working on a ladder jack scaffold approximately 10ft above the ground. The 2"x12"x12' platform extended over the support approximately 6ft. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: a) Worksite, 39 Kingsbury Rd. Garden City NY - Employees were using a Makita Circular Saw, Model#5007F, Serial #0252534, to cut lumber with the guard wedged in the open position; on or about 8/8/15. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19. Recommendation Contracting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.304(d), which was contained in OSHA inspection number 917312, citation number 1, item number 3, and was affirmed as a final order on 3/24/14, with respect to a workplace located at 500 W. Penn St. Long Beach, NY 11561.
