29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Staging/Processing Area - On or about 08/06/2015, ground men assisting in the process of felling a tree were exposed to a struck by hazard, for they were not provided with eye protection while operating a chipper and chain saws. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
