29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE).  If such hazards were present, or likely to be present, the employer did not: (i) Select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and (iii) Select PPE that properly fit each affected employee:  a.   On or about May 1, 2015, an employee working on the rooftop of a single story residential house was not provided the proper personal protective equipment to protect the employee from a 10 feet fall hazard from the unprotected edge of the roof.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   a.  On or about May 1, 2015, the employer failed to inform OSHA within 24 hours after the hospitalization of one of their employees from a work related accident.  The employer was advised of the Victim's accident around 11:00 am EST on May 1, 2015.
