On or about July 30, 2015, the scaffold at south building wall had no ladder, stair, ramp, walkway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was used to access a scaffold platform that was greater than 2 feet above the surrounding pavement.  Two employees worked from the scaffold platform which exposed them to fall hazards.
