29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      The employer does not provide eye and face protective equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. This violation occurred on or about August 6, 2015, on the north side of a roof when employees were exposed to a struck by hazard when operating nail guns to install roof shingles without wearing eye protection.

This violation occurred on or about August 6, 2015 on the north side of a roof when employees were exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    The employer does not ensure that the side rails on portable ladders used for access to an upper landing surface were extended at least 3 feet above the upper landing surface being accessed.  This violation occurred on or about August 6, 2015 on the north side of a roof where employees were exposed to a fall hazard when accessing the roof without the side rails extending at least 3 feet above the landing surface.
