An extension platform onto which materials can be hoisted for handling a) At the trash compactor area, on or about 8/6/2015: The wall opening for the trash compactor chute, which moved trash down chute into the trash compactor, lacked adequate guards or barriers to protect the employee dumping trays of vials while leaning into the chute. Abatement certification must be submitted.
