Site - On or about August 7, 2015, the red fiberglass extension ladder in use to access the roof area did not extend 3 feet above the upper landing and working surface.     Instance b. Site - On or about August 7, 2015, the 20 foot aluminum ladder in use to access the roof area did not extend 3 feet above the upper landing and working surface.    Instance c. Site - On or about August 7, 2015, the aluminum ladder in use to access the ladder scaffolding system did not extend 3 feet above the upper landing and working surface.
