29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet or less in width, each employee was not protected by use of a monitoring system: a) Perfect Roofing at 1230 W. 38th Ave., Denver, CO 80211: On or before August 8, 2015, the employer did not ensure that employees were protected from falling when laying a Firestone TPO roof in that the safety monitor was engaged in roofing activities when the other employees were working near the roof edge. In addition, there were no flags set up on the west end where the employees were working. Condition exposed employees to fall hazards of up to 18 feet.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59) a) Perfect Roofing at 1230 W. 38th Ave., Denver, CO 80211: On or before August 8, 2015, the employer did not ensure that employees were trained regarding the chemical hazards of the workplace including the use of the Firestone bonding adhesive W56-TPO-3005, which contained solvents including Acetone, Hexane, Toluene, and Xylene. This condition potentially exposed employees to chemical hazards..
