29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite - Employees were not protected from falling through holes on the walking working surface while working on the third and fourth levels. This condition exposed employees to a fall hazard of approximately ten (10) to twelve (12) feet to the next lowest level. Observed on or about 18 MAY 2015.
